PFA has been a game-changer for heart patients—but there are still risks

Pulsed field ablation, or PFA, has quickly become a go-to treatment for patients with atrial fibrillation, offering a faster and safer alternative to older methods like radiofrequency or cryoablation. But as more hospitals adopt the technology, cardiologists are also learning about its uncommon but serious complications.

Dr. Devi Nair, a cardiac electrophysiologist, recently reviewed a range of rare adverse events linked to PFA. While the procedure is praised for its ability to target heart tissue without damaging nearby structures like the esophagus or lungs, it is not risk-free. Nair pointed out that some patients have experienced silent cerebral emboli, where tiny clots travel to the brain without immediate symptoms. Others have developed pericardial effusion, a buildup of fluid around the heart that can become life-threatening if not caught early.

Another concern involves coronary artery spasm, which can mimic a heart attack during or after the procedure. Nair also noted cases of phrenic nerve injury, though these appear less frequent than with older ablation techniques. The key, she emphasized, is that these complications remain rare, and PFA still represents a major step forward for many patients who previously had limited options.

Still, Nair urged clinicians to stay vigilant. As PFA devices become more common in hospitals, doctors must carefully screen patients and monitor them closely during and after the procedure. The technology is a game-changer, she said, but it demands respect for the unknowns that come with any new tool in medicine. For now, the benefits outweigh the risks for most heart patients, but the learning curve continues.
